Summary

Summary:

  • The Middle East remains the epicentre of global conflict, with the Gaza ceasefire under severe strain β€” over 72,500 Palestinians killed since October 2023 β€” and a new EU/UN assessment estimating $71 billion needed for Gaza's recovery over the next decade.
  • The fragile US-Iran ceasefire is set to expire Wednesday evening Washington time; President Trump has threatened to destroy Iranian infrastructure if talks fail, while Iran has denied it will attend a second round of negotiations in Islamabad.
  • In Lebanon, a 10-day ceasefire between Israel and Hezbollah continues to hold, though a UN peacekeeper was killed at the weekend and thousands of displaced civilians are returning to heavily damaged southern towns.
  • Ukraine's war enters its fifth year with no ceasefire in sight: 206 frontline clashes were recorded on 20 April, Russia launched its deadliest aerial attack of 2026 on 15–16 April killing at least 20 civilians, and the UN has verified 15,578 Ukrainian civilian deaths since February 2022.
  • Sudan's civil war marks three years of catastrophe: at least 59,000 killed, 14 million displaced, and the UN describes it as the world's largest humanitarian crisis, yet it remains severely underfunded and underreported.
  • Myanmar's military junta continues airstrikes on civilian targets in Rakhine and Chin states, with a bomb landing on Thai territory on 20 April, prompting evacuations in Mae Hong Son province.
  • In the Democratic Republic of Congo, the government and M23 rebels signed a humanitarian access protocol in Montreux, Switzerland, though fighting and civilian casualties continue in eastern DRC.
  • Haiti faces a security vacuum as the final Kenyan police contingent of the UN-backed multinational mission withdrew, with civilians taking to the streets to protest amid fears of a full return of gang violence.

1. Gaza and the Occupied Palestinian Territory

The death toll from Israel's military campaign in Gaza since 7 October 2023 has risen to at least 72,553, with 172,296 people wounded, according to Gaza's Ministry of Health. Middle East Eye; Anadolu Agency

Israeli fire killed at least two Palestinians in separate incidents on 20 April. Gaza's Government Media Office states Israel has committed 2,400 violations of the October 2025 ceasefire agreement, including killings, arrests, blockades and starvation policies. At least 777 people have been killed since the ceasefire took effect, including 32 since the start of April. Al Jazeera

A landmark joint EU and UN Gaza Rapid Damage and Needs Assessment (RDNA), released on 20 April, found that more than $71 billion will be needed over the next decade for Gaza's recovery and reconstruction. Physical infrastructure damages alone are estimated at $35.2 billion, with economic and social losses of $22.7 billion. The Israeli bombardment has generated over 61 million tonnes of rubble; 371,888 housing units have been destroyed or damaged; more than 50% of hospitals are non-functional; and nearly all schools have been destroyed or damaged. Gaza's economy has contracted by 84%, and 1.9 million people have been displaced. The conflict has set back human development in Gaza by 77 years. UN Office of the Special Coordinator for the Middle East Peace Process

UN Women reported on 20 April that an average of at least 47 Palestinian women and girls were killed daily during Israel's war on Gaza, totalling over 38,000 deaths between October 2023 and the end of 2025. A further 11,000 have been injured, many with life-altering disabilities. UN Women

Over 60 nations gathered in Brussels on 20 April for a EU-hosted Palestinian peace conference to discuss stability, security and long-term peace in Gaza and the occupied West Bank. EU foreign policy chief Kaja Kallas, Norwegian Foreign Minister Espen Barth Eide, and Palestinian Prime Minister Mohammad Mustafa addressed the conference. The EU and UN called for Gaza's reconstruction to be "Palestinian-led" and based on a transition of governance to the Palestinian Authority. Al Jazeera

2. US-Iran War and the Strait of Hormuz Crisis

The fragile two-week ceasefire between the United States and Iran is due to expire on Wednesday evening Washington time. President Trump announced on Sunday that a second round of US-Iran peace talks would take place in Islamabad, Pakistan, on Monday β€” but Iran denied it would attend, accusing the US of "armed piracy" after US forces intercepted and seized an Iranian-flagged tanker, the Touska, in the Gulf of Oman. Al Jazeera

Trump threatened to "knock out every single power plant, and every single bridge, in Iran" if Tehran does not accept US terms. Experts have warned such strikes could constitute war crimes under international law. Iran's Khatam al-Anbiya military headquarters confirmed the US attack on the tanker and said it would "respond soon"; Iranian forces subsequently sent drones toward US military ships. Al Jazeera; NPR

The key sticking points in negotiations include: Iran's insistence on sovereignty over the Strait of Hormuz; US demands for zero uranium enrichment (which Iran rejects as a violation of its NPT rights); Iran's refusal to discuss its ballistic missile programme; and Iran's continued support for regional allies including Hezbollah, the Houthis and Iraqi militias. Shipping traffic through the Strait of Hormuz has fallen by 95% since the war began on 28 February 2026. Al Jazeera

Yemen's Iran-aligned Houthi movement has threatened to close the Bab al-Mandab Strait, a second critical global shipping route, amid the escalating rhetoric. Oil prices have surged as markets price in the risk of a wider supply disruption. Gulf News

Pakistan has been playing an active mediating role. A UK-France multinational mission to protect the Strait of Hormuz was announced at a Paris summit on 17 April. China-US Focus

3. Lebanon β€” Fragile Ceasefire

A 10-day ceasefire between Israel and Hezbollah continues to hold as of 20 April, with thousands of displaced Lebanese civilians returning to heavily damaged towns in southern Lebanon. However, a UN peacekeeper was killed over the weekend, likely from Hezbollah fire, according to UN News. Israel has established a "yellow line" in southern Lebanon demarcating its occupation zone, and has warned Lebanese civilians against returning to certain areas. UN News; Al Jazeera

The US confirmed a second round of direct Israel-Lebanon negotiations will take place Thursday. Lebanon's Cash Working Group reported that since early March 2026, the country has experienced rapid deterioration in humanitarian conditions, with large-scale displacement exceeding 1 million people. 4. Ukraine β€” War Enters Fifth Year

Ukraine's war against Russia's full-scale invasion entered its fifth year with no ceasefire agreement in sight. On 20 April, 206 frontline combat engagements were recorded, with the fiercest fighting in the Pokrovsk and Huliaipole sectors. Russia used over 7,700 drones and launched 71 airstrikes over the preceding 24 hours. Ukrinform (Ukr)

Ukraine's military intelligence claimed successful drone attacks on two Russian landing ships and a radar station in Sevastopol Bay, Crimea, on 20 April. Ukrainian drones also struck the port of Tuapse in Russia's Krasnodar region, killing at least one person and damaging transport infrastructure. Al Jazeera

Russia launched its deadliest aerial attack of 2026 on the night of 15–16 April, targeting Odesa, Dnipropetrovsk, Kyiv, Sumy and Kharkiv regions. At least 20 civilians, including one child, were killed, and dozens more injured. Residential buildings in Odesa and Dnipro were hit. In March 2026 alone, at least 211 civilians were killed and 1,206 injured. UN DPPA / Security Council

Since February 2022, OHCHR has verified at least 15,578 Ukrainian civilians killed, including 784 children, and 43,352 injured, including 2,668 children. Ukraine's Ministry of Defence estimated total Russian combat losses at approximately 1,319,270 personnel as of 20 April 2026. UN DPPA; Ukrinform (Ukr)

Russia's Easter truce (11–12 April) was not respected, with hostilities and civilian casualties reported throughout the holiday weekend. The 72nd prisoner exchange since the start of the war took place on 11 April, returning 175 service members and seven civilians from each side. UN DPPA

Trump called for a 30-day ceasefire in the Russia-Ukraine war; Ukraine agreed, but the Kremlin has not given a clear answer. Peace negotiations have been further stalled since the outbreak of the US-Iran war. Democracy Now!

5. Sudan β€” Three Years of War

Sudan's civil war marked its third anniversary on 15 April 2026, with the conflict now widely described as the world's largest humanitarian crisis. At least 59,000 people have been killed, according to the Armed Conflict Location and Event Data Project (ACLED). An estimated 14 million people have been displaced β€” the world's largest displacement crisis β€” and 21 million face starvation. OHCHR; Asharq Al-Awsat

UN Human Rights Chief Volker TΓΌrk, following a recent visit to Sudan, described the situation as "a human rights catastrophe unfolding in real time," citing accounts of extreme brutality including atrocity crimes, gang rape, and torture used as a weapon of war. At least 175 health workers have been killed and 183 health facilities damaged or destroyed since the conflict began. OHCHR; ReliefWeb

In Rakhine State, airstrikes killed one person and injured at least 14, including monks studying at a monastery. In Chin State, twin airstrikes struck Tlangzar Village in Falam Township, damaging multiple homes. Myanmar Now; BNI Online

In a significant escalation, a bomb from a Myanmar Air Force airstrike near the Thai border landed on Thai territory in Mae Hong Son's Sop Moei district on 20 April, prompting local evacuations. The Arakan Army (AA) reported the junta has stepped up naval and drone operations in southern Rakhine State. Straits Times; Irrawaddy

Analysis from ABC News suggests anti-junta resistance forces may be gaining the upper hand in the broader civil war, though the conflict remains at a stalemate in many areas. ABC News

7. Democratic Republic of Congo β€” Humanitarian Access Deal

Following peace talks in Montreux, Switzerland, the Congolese Government and the AFC/M23 signed a protocol on humanitarian access and judicial protection on 20 April. The UN peacekeeping mission MONUSCO described the signing as "an important step toward rapid, safe, sustained and unimpeded humanitarian access." A separate memorandum was signed to operationalise the Expanded Joint Verification Mechanism Plus, aimed at strengthening monitoring on the ground. UN News; RFI

Despite the diplomatic progress, the DRC army accused M23 rebels of deadly attacks during the talks, with a provisional toll of 4 civilians killed and 8 injured. Anadolu Agency
